I acknowledge and understand that participation in fitness classes and related activities conducted by PLUSONE involves certain risks and potential dangers. I voluntarily agree to participate in the class and assume all risks associated with my participation.

I acknowledge that I am voluntarily participating in the fitness class(es) provided by the Provider. I understand that physical exercise, by its very nature, carries with it certain inherent risks, including but not limited to physical injury, strain, discomfort, and even the possibility of serious injury except for death or personal injury arising through our negligence, I hereby assume all risks and responsibility for any such injuries or other medical incidents.

I hereby release, waive, discharge, and agree not to sue the Provider, its employees, representatives, affiliates, or agents from any claims, demands, liabilities, rights, damages, expenses, and causes of action of any nature arising out of or in connection with my participation in the fitness class(es),

I represent that I am physically fit to participate in the fitness class(es) and have no medical condition that would prevent my safe participation. If I have any medical conditions or concerns, I have consulted with a healthcare provider and obtained clearance to participate.

 

By bringing my child/children with me to the fitness class, I confirm that:

I am the Parent or Legal guardian of the child(ren)

I understand and will ensure that the child(ren) entered will be accompanied and supervised by myself at all times

I consent for my child(ren) to use the playground equipment and I understand the inherent risks associated

I, for myself and for my child(ren) fully aware of the risks associated with participation in activities and use of facilities and equipment. These risks include damage to or loss of personal property, personal injury and in extreme cases, death.

I and my child(ren) will comply with all instructions given to me by fitness instructors, officers, employees and volunteers during the fitness class and use any equipment as directed and not so as to hurt or injure others.

I acknowledge that the Child/Children are sufficiently fit and healthy to safely participate and do not suffer from any medical condition which would adversely affect their ability to safely participate in activity.

I, for myself and for my child(ren) hereby acknowledge and accept that the fitness instructors, officers, employees and volunteers shall not be liable for any loss or damage arising from the Child(ren)’s participation in the fitness class , including indirect or consequential loss or damage.

 

PHOTOGRAPH WAIVER FORM

I give consent for myself or my child to be photographed, videotaped or filmed while participating in activities and for the resulting images to be used by for promotional purposes. This release is mandatory for volunteering.
